Please note that this is the process for citation and proclamation requests in District 37. If your honoree is not based in Council Member Nurse’s district, please submit a request to your local Council Member. You can find their contact information here.
Please complete this Citation & Proclamation Request Form at least three weeks prior to the date of the event to ensure timely processing. All requests are subject to review and approval.
Citations
For special events, the Council is able to recognize individuals and organizations from District 37 who provide an exemplary service to their community, whether through unique personal achievement in their professional or other endeavors or simply through a lifetime of philanthropy and community service.
Proclamations
The Council may bestow proclamations upon community leaders or non-profit organizations for their stellar service to the community, and/or for their outstanding achievements. These are typically presented at special events, which can include Milestone Anniversaries (those ending in 0 or 5) and Retirements, but may also include Birthdays (for honorees 75+) and a range of other special occasions.
Proclamation requests must be submitted at least three weeks prior to the date of the event. If a request is submitted within less time, you must also submit a proclamation draft (as an attachment) and there is no guarantee that it will be finished on time for your event.
